Job summary

RHA Health Services, LLC is hiring a CAP Case Manager in the New Hanover & Brunswick area to provide essential case management for beneficiaries at risk of institutionalization. The role offers flexible hours and requires coordination with RNs and an interdisciplinary team to support person-centered care planning and service integration.

Responsibilities include assessments, care planning, service coordination, and ongoing monitoring to ensure health and safety.

Qualifications

• Bachelor’s degree in social work or related field required (NC Medicaid-certified training within 90 days is expected).
• Experience with Medicaid/case management in health or community settings preferred.
• Ability to communicate clearly with families and multidisciplinary teams.

Responsibilities

• Conduct initial assessments to evaluate medical, behavioral, financial, social, and environmental needs.
• Coordinate care plans and education about waiver services and community resources.
• Monitor progress through monthly contacts and home visits; document outcomes and adjust plans as needed.
• Link beneficiaries to services, coordinate with providers, and ensure compliant documentation.
