Job summary

RHA Health Services, LLC is seeking a CAP Case Manager / Social Worker / PT in Gaston County with flexible hours. The role focuses on initial assessment, care planning, coordination of services, and ongoing monitoring to support beneficiaries at home.

The position requires a social work degree, Medicaid training within 90 days, and the ability to collaborate with an interdisciplinary team to ensure high-quality, person-centered care.

Qualifications

• Bachelor’s degree in social work from an accredited school of social work.
• Minimum one year of related community experience, preferably case management.
• Completion of an NC Medicaid-certified training program within 90 days.

Responsibilities

• Assess beneficiaries and families to evaluate needs across medical, social, and environmental areas.
• Coordinate services and referrals to support home care and community resources.
• Monitor progress via monthly calls and home visits, updating care plans as needed.
• Educate beneficiaries and families about plan of care and available resources.
• Ensure documentation and regulatory compliance in all records.
• Collaborate with RNs and the interdisciplinary team for quality care.
