Enlyte in California is seeking a Field Case Manager (RN) for a full-time hybrid role, requiring local travel to in-person appointments in the Santa Maria area.

You will collaborate with treating physicians, employers, and patients to develop and implement treatment plans that return the injured worker to work safely, ensure cost-effective care, and monitor recovery progress. Strong independence, communication, and organizational skills are essential.

Competenze

• Associates or Bachelor’s in Nursing or related field.
• Active RN license in good standing.
• URAC-recognized case management certification (CCM/CDMS/CRC/CRRN/COHN/COHN-S/RN-BC/ACM/CMAC/CMC).
• Travel to in-person appointments within assigned geography.
• Reliable internet access and transportation.

Mansioni

• Demonstrate knowledge and application of case management standards.
• Advise on treatment plans based on injuries and job duties.
• Interview patients to assess recovery and occupational vs non-occupational factors.
• Collaborate with physicians to tailor treatment plans.
• Modify job duties with employers to facilitate early return to work.
• Manage workload independently and provide status updates to stakeholders.
