Enlyte is seeking a Field Case Manager (RN) in the Sacramento, CA area to coordinate care for injured workers and clients, with regular local travel to in-person appointments. This full-time hybrid role combines clinical expertise with advocacy to return employees to work safely and cost-effectively.

You will collaborate with physicians, employers and patients, manage caseloads, document updates, and participate in a productivity-based bonus program.

Qualifications

• Active RN license required in good standing.
• 2+ years of clinical practice preferred.
• URAC-certified case management credentials encouraged.

Responsabilités

• Apply case management standards of practice.
• Advise on treatment plans with physicians and employers.
• Interview injured workers to assess recovery.
• Collaborate with physicians to tailor Plans.
• Modify job duties to facilitate early return to work.
• Manage workload and priorities independently.
• Attend appointments and provide status updates.
