Job summary

Enlyte is seeking a Field Case Manager in the San Jose, CA area to coordinate treatment plans between physicians, employers, and patients. This full-time, hybrid role requires travel for in-person appointments and offers comprehensive benefits and a balanced workload designed to restore patients to work.

The ideal candidate holds an active RN license, has 2+ years of clinical experience, and can advocate effectively for optimal recovery and cost-effective care.

Qualifications

• Active RN license in good standing.
• 2+ years clinical practice preferred; workers' compensation experience preferred.

Responsibilities

• Demonstrate knowledge, skills, and competency in the application of case management standards of practice.
• Advise on treatment plans considering injury type, medications, comorbidities, job duties.
• Interview disabled persons to assess recovery and whether injuries are occupational or non-occupational.
• Collaborate with treating physicians/providers to create and implement individualized treatment plans.
• Work with employers and physicians to modify job duties to facilitate early return to work.
• Evaluate and modify case goals based on patient progress and treatment effectiveness.
• Manage workload independently, prioritizing cases and ensuring efficient case handling.
