Job summary

Community Counseling of Bristol County seeks a Coordinated Entry Specialist for the New Bedford Continuum of Care to locate, assess, and refer individuals experiencing homelessness to housing services. You will work with a small team to maintain a list of people in our area and coordinate with agencies to move them toward housing quickly.

Strongly prefer multilingual candidates (Spanish, Portuguese, Haitian Creole, Cape Verdean Creole) and you will triage with partners to divert those at risk of

Qualifications

• 3-5 years of experience working with homeless populations or homeless programs.
• Knowledge of available referral sources for services to clients.
• Ability to drive throughout the county and transport clients.
• Ability to interact with people from diverse backgrounds with respect and compassion.
• Strong written and oral communication; follow detailed instructions.
• Comfortable using Microsoft Office 365 tools.

Responsibilities

• Coordinate with client, housing and homeless agencies and programs to house and stabilize those in a housing crisis.
• Refer qualifying clients to housing programs and services to establish housing and needed services.
• Train agencies and community partners on the Coordinated Entry system and how to help those in a housing crisis.
• Divert clients facing homelessness to resources including their own support networks.
• Maintain a By-Name list of homeless individuals to coordinate efforts and shorten homelessness episodes.
