COMMUNITY ACTION PIONEER VALLEY in Greenfield, MA seeks a COC Coordinated Entry Coordinator for 37.5 hours/week, hybrid work arrangement. • 4 years' experience in human services with vulnerable populations.
• Experience with data management and outcomes analysis.

Responsibilities

• Lead Coordinated Entry across Berkshire, Franklin, Hampshire and North Quabbin regions.
• Oversee day-to-day operations, implementation and continuous improvement.
• Facilitate case conferencing and coordinate housing resources access.
• Provide training and technical assistance to partners and staff.
• Engage with providers, people with lived experience, and agencies to remove barriers.
