Job summary

Symba Center is seeking a Coordinated Entry Case Manager in Victorville, CA to guide individuals through the CES, assess housing needs, complete eligibility verifications, and connect clients with housing and benefits. The role involves collaborating with landlords and partner agencies to remove barriers to housing stability.

You will document, coordinate, and advocate for clients while supporting health and social services integration, including managing risk and confidentiality per HIPAA

Qualifications

• High School Diploma or equivalent required.
• 2–3 years of housing navigation, case management, or homeless services experience preferred.
• Knowledge of Coordinated Entry Systems and housing resources.
• Valid, unrestricted California driver’s license and clean DMV record.
• BLS certification from AHA or Red Cross preferred.

Responsibilities

• Conduct comprehensive housing assessments and verifications through the Coordinated Entry System.
• Maintain up‑to‑date documentation in line with CES policies.
• Assist clients with individualized housing plans and referrals.
• Help clients locate and apply for housing and public benefits, gathering required documents.
• Coordinate with landlords, property managers, and partner agencies to facilitate housing opportunities.
• Maintain confidential records in HMIS and ensure HIPAA compliance.
