Job summary

Community Counseling of Bristol County (CCBC) is hiring full-time Community Outreach Case Workers to join the Community Support Program in Bristol and Plymouth counties. The role provides short-term, intensive case management to clients with behavioral health and/or substance use histories, helping them reach goals and live independently.

Strong preference for multilingual candidates (Spanish, Portuguese, Haitian Creole, or Cape Verdean Creole). Hybrid work option available once trained.

Qualifications

• Bachelor's degree in psychology, social work, sociology, or related field.
• Alternatively, a minimum of 3 years of related experience.
• Strong time management and communication abilities required.

Responsibilities

• Establish and maintain relationships with clients through outreach in the community, home, or designated locations.
• Collaborate with internal and external team members and natural supports.
• Conduct comprehensive Needs Assessments and create Individualized Action Plans (IAP).
• Connect clients to benefits, housing, health care and other resources.
• Provide transportation to medical and behavioral health appointments when needed.
• Foster empowerment, recovery, and wellness with peer supports and self-help services.
• Document encounters and maintain client electronic medical records.
• Obtain insurance authorizations and check eligibility daily.
• Participate in staff meetings, trainings, and agency events.
