Job summary

South Middlesex Opportunity Council, Inc. is seeking a Street Outreach/CE Case Manager in Framingham, MA. You will provide street outreach, perform comprehensive needs assessments, and engage clients toward coordinated entry and housing resources.

Travel throughout the western Massachusetts region is required. You will work with partner agencies to ensure safety, health, and access to services, while maintaining confidential client records.

Qualifications

• Bachelor’s degree or 3+ years of experience in human services or related field.
• Ability to engage diverse client populations with empathy and professionalism.
• Flexible schedule and willingness to travel regionally.

Responsibilities

• Provide street outreach and implement engagement strategies with new clients.
• Develop, implement, and monitor individualized service plans for participants.
• Coordinate with CE housing navigators and other providers to ensure service delivery.
• Maintain confidential client records and submit monthly updates.
• Travel throughout the CE west region as required.
