Job summary

Community Action Pioneer Valley, Inc. seeks a Coordinated Entry (CE) Coordinator to lead the Three County CoC CE system across Berkshire, Franklin, Hampshire, and North Quabbin regions from Greenfield, MA.

You will oversee access, assessment, eligibility, prioritization, referrals, and housing placements, while training providers and ensuring HUD compliance. The role emphasizes trauma-informed, equity-focused practices, using HMIS and data to monitor performance and drive continuous improvement

Qualifications

• 4 years' experience in human services or related field
• Experience with data management and HMIS desired
• Strong communication and organizational skills

Responsibilities

• Manage and coordinate day-to-day operations of the Three County CoC Coordinated Entry System, including access, assessment, eligibility, prioritization, referral, housing placement, and connection to resources across Berkshire, Franklin, Hampshire Counties and the North Quabbin region.
• Facilitate Coordinated Entry case conferencing meetings across all three counties and support timely, equitable referrals and housing placements.
• Serve as primary point of contact for Coordinated Entry questions, referrals, troubleshooting, and system navigation; escalate issues to the Program Director as appropriate.
• Maintain By-Names-List and current knowledge of available housing resources; coordinate with housing providers and partners to support referrals and improve housing outcomes.
• Develop and provide training and ongoing support to Coordinated Entry providers, assessors, partners, and system users on policies, procedures, HMIS, and related requirements.
• Identify training and TA needs and work with leadership to develop system-wide solutions, tools, and resources to strengthen provider capacity.
