Qualifications

• Requires a Bachelor's degree and two years' experience in social services for individuals with developmental disabilities.
• Alternative: six years of relevant social services experience.
• Equivalent education/experience considered.
• Graduate training can substitute for one year of experience.
• Not entry-level; prior professional case management experience is required.

Responsibilities

• Identify client needs, plan, and authorize payment for services.
• Schedule and conduct home visits with clients and families.
• Facilitate person-centered goal planning and update service plans.
• Complete client assessments and plans within established timelines.
• Determine and monitor client eligibility for programs.
• Coordinate with state agencies and community partners.
• Maintain complete case files and payment records.
• Refer clients in crisis to appropriate authorities.
• Collaborate with community partners and agencies.
• Represent DDA professionally at meetings.
