Job summary

CVS Health is seeking a skilled social work professional to coordinate long-term care and psychosocial services for members. You will lead case management across settings, collaborate with care teams, and implement strategies to improve outcomes while managing costs.

The role requires strong communication, cultural sensitivity, and the ability to travel regionally. A Bachelor’s in Social Work and state licensure are required.

Qualifications

• Minimum 2 years in medical social work or case management.
• Must meet state requirements where duties are performed.
• Travel up to 25–50% in Wayne and Macomb Counties with reliable transportation.

Responsibilities

• Manage care for members with acute or chronic conditions focusing on quality, outcomes and costs.
• Perform initial and periodic assessments for Long-Term Care and case management programs.
• Develop individualized case plans for at‑risk members in disease management.
• Communicate with providers, primary care physicians, Medical Directors and care teams.
• Ensure members receive care covered by their benefit plan in home/community settings.
• Provide information and education on social issues, mental health and services.
• Develop programs to reduce admissions and improve health outcomes.
• Coach junior staff in social work techniques and processes.
• Collaborate with community groups to address issues and raise awareness.
