Job summary

CVS Health is seeking a Care Coordinator of Social Work to manage members with chronic conditions across the continuum of care. You will develop individualized case plans, coordinate with providers, and leverage community resources to prevent readmissions and promote quality of life.

Role requires a Master’s in Social Work, LMSW, and willingness to travel 25–50% in Wayne and Macomb Counties. Excellent collaboration and communication skills are essential.

Qualifications

• Master's Degree in Social Work required by state law.
• LMSW licensure required or in place where applicable.
• Minimum years experience in medical social work or case management.
• Willingness to travel up to 25%–50% in specified counties; reliable transportation.

Responsibilities

• Coordinate care for members with long-term rehabilitation and psychosocial needs.
• Implement and coordinate case management, psychosocial, and quality of life activities.
• Develop and carry out strategies to integrate acute and long-term care services.
• Provide information and education on social issues and available services.
• Coach junior colleagues in social work techniques and processes.
• Collaborate with community members and policymakers to address issues.
