Qualifications

Responsibilities

• Coordinate care for members with long-term rehabilitation or psychosocial needs to reduce readmissions.
• Develop and implement case management plans across care settings.
• Communicate with providers, care managers, and physicians about patient status.
• Identify opportunities to maximize care within the member's benefit plan.
• Provide information and education on social issues and available services.
• Develop programs to reduce hospital admissions and improve community health outcomes.
• Coach junior colleagues in social work techniques and processes.
• Collaborate with community organizations and policymakers to address client needs.
