CVS Health is seeking a qualified care coordinator to support members with long‑term rehabilitation and psychosocial needs. You will manage cases across the continuum of care, coordinate referrals, and help families access home and community resources to improve outcomes and reduce readmissions.

Responsibilities include developing individualized care plans, communicating with providers and care teams, and guiding members through available services.

Competenze

• Bachelor's degree in Social Work required by state law.
• LLBSW, LBSW or LLMSW licensure required where duties performed.
• Minimum 2 years in medical social work or case management.
• Willingness to travel up to 25–50% in Wayne and Macomb Counties.

Mansioni

• Develop and carry out strategies to coordinate acute and long‑term care services.
