CVS Health is seeking a dedicated care coordinator to manage members with long-term rehabilitation and psychosocial needs. You will coordinate all case management activities across the continuum of care, including home care visits, resource identification and consultant referrals.

You will work with providers, care managers and communities to develop integrated care plans and reduce unnecessary admissions while promoting quality of life.

Qualifikationen

• Minimum 2 years experience in medical social work or case management.
• Willing to travel up to 25-50% in Wayne and Macomb Counties; reliable transportation.
• Bachelor's degree in Social Work; state licensure requirements may apply.
• Proficient in problem solving, digital literacy and medical terminology.
