Qualifications

• Minimum years experience in medical social work or case management.
• Must meet state requirements where duties are performed.
• Willingness to travel up to 25%–50% in Wayne and Macomb Counties with reliable transportation.
• Problem solving and decision making skills.
• Digital literacy skills and knowledge of medical terminology.
• Ability to handle sensitive information ethically and responsibly.
• Clinical setting with diverse cultural dynamics.

Responsibilities

• Coordinate care for members with acute or chronic conditions to improve outcomes and reduce costs.
• Perform initial and periodic assessments within case or disease management programs.
• Develop individualized care plans for at-risk members.
• Communicate with providers, PCPs, Medical Directors, pharmacists, and staff on patient status.
• Identify opportunities to ensure care aligns with benefit plans in home or community settings.
• Provide information and education on social issues, mental health, and available services.
• Develop programs to reduce admissions and improve community health outcomes.
• Mentor junior colleagues in social work techniques and processes.
• Collaborate with community members, organizations, and policymakers on issues affecting clients.
