Qualifications

• Minimum 2 years experience in medical social work or case management.
• Must comply with all state requirements in the state where job duties are performed.
• Willing and able to travel up to 25%-50% in Wayne and Macomb Counties; mileage reimbursed.

Responsibilities

• Controls comprehensive management of members with acute or chronic conditions; focus on quality of care, outcomes and costs.
• Conducts initial and periodic assessments for Long-Term Care Program and case/disease management.
• Develops individualized case plans for at-risk members.
• Communicates with providers, primary care physicians, Medical Directors, pharmacists, and care management staff.
• Identifies opportunities to ensure members receive care covered by their benefit plan.
