Qualifications

• 2+ years in medical social work or case management.
• Must travel up to 25%-50% in Wayne and Macomb Counties; reliable transportation.
• Complies with state requirements for the job duties.
• Basic problem solving and decision making skills.
• Basic digital literacy and medical terminology knowledge.
• Ability to work tactfully with customers and community.
• Ethical handling of sensitive information.
• Ability to function in clinical settings with diverse cultural dynamics.

Responsibilities

• Coordinate care for members with long-term rehabilitation or psychosocial needs to prevent readmissions.
• Conduct initial and periodic assessments for the Long-Term Care program and case management.
• Develop individualized care plans and coordinate services across care continuum.
• Communicate updates with providers, physicians, pharmacists, and care management staff.
• Identify opportunities to ensure members receive care under their benefit plan in home or community settings.
• Provide information, resources, and education on social issues and mental health.
• Develop programs to reduce admissions and improve health outcomes.
• Coach junior colleagues in social work techniques and responsibilities.
• Collaborate with community members and policymakers to address issues.
