Job summary

You will develop and carry out strategies to integrate acute and long-term care services, provide psychosocial interventions, and work with diverse clinical teams to support patients across settings.

Qualifications

• Minimum 2 years experience in medical social work or case management.
• Bachelor's Degree in Social Work and licensure (LLBSW/LBSW/LLMSW) required.
• Willingness to travel 25–50% in Wayne and Macomb Counties; reliable transportation.
• Must comply with state requirements where job duties are performed.
• Basic digital literacy and problem-solving skills.

Responsibilities

• Controls comprehensive management of members with acute or chronic conditions focusing on quality of care and outcomes.
• Performs initial and periodic assessments for Long-Term Care or case/disease management programs.
• Develops individualized case plans for at-risk members within case or disease management.
• Communicates updates with providers, PCPs, Medical Directors, pharmacists, and care management staff.
• Identifies opportunities to ensure members receive care covered by their benefit plan at home or in the community.
• Provides information and resources on social issues, mental health and available services.
• Develops programs to reduce admissions and improve community health outcomes.
• Coaches junior colleagues in social work techniques and processes.
• Collaborates with community organizations and policymakers to address issues and raise awareness.
