Job summary

CVS Health is seeking a Care Coordination professional to manage long-term rehabilitation and psychosocial needs for members, coordinating case management and social work services across care settings.

You will implement strategies, liaise with providers and care teams, and develop programs to improve outcomes and reduce hospitalizations. Travel within Wayne/Macomb Counties is required, with a 40-hour work week and full-time schedule.

Qualifications

• Bachelor's Degree in Social Work required by state law.
• LLBSW, LBSW, LLMSW licensure preferred.
• Minimum 2 years in medical social work or case management.
• Willing to travel 25–50% in Wayne and Macomb Counties with reliable transportation.
• Basic digital literacy and medical terminology knowledge.
• Ability to handle sensitive information ethically.

Responsibilities

• Controls comprehensive management of members with acute or chronic conditions, focusing on quality of care, outcomes and cost reduction.
• Performs initial and periodic assessments for long-term care or case/disease management programs.
• Develops individualized case plans for at-risk members.
• Communicates updates with providers, primary care physicians, Medical Directors, pharmacists, and care management staff.
• Identifies opportunities to ensure members receive necessary care under their benefit plan.
• Provides information and education on social issues and available services.
• Develops programs to reduce admissions and improve health outcomes.
• Coaches junior colleagues in social work techniques and processes.
• Collaborates with community members and policymakers to address issues and raise awareness.
