Qualifications

• Minimum 2 years experience in medical social work or case management.
• Willing to travel up to 25%-50% in Wayne and Macomb Counties.
• Reliable transportation required.
• Must meet state licensure requirements.
• Basic awareness of problem solving and decision making skills.
• Basic awareness of digital literacy skills.
• Basic knowledge of medical terminology.
• Ability to deal tactfully with customers and community.
• Ability to handle sensitive information ethically and responsibly.

Responsibilities

• Coordinate care for members with long-term rehabilitation or psychosocial needs.
• Implement and coordinate all case management and quality of life activities.
• Develop and carry out strategies to coordinate care across acute and long-term services.
• Provide information and education on social issues and available services.
• Develop programs to reduce hospital admissions and improve health outcomes.
• Coach junior colleagues in social work techniques and processes.
• Collaborate with community members and organizations to address issues.
