Responsibilities

• Coordinates care for members with long-term rehabilitation or psychosocial needs to prevent adverse outcomes.
• Performs assessments for Long-Term Care and disease management programs.
• Develops individualized case plans and coordinates services across care continuum.
• Communicates with providers, physicians, pharmacists and care management staff regarding patient status.
• Identifies opportunities to ensure members receive care covered by their benefit plans.
