Job summary

Qualifications

• Master's Degree in Social Work required by state law.
• LMSW certification required or preferred.
• Willing to travel up to 25–50% in Wayne and Macomb Counties.
• Experience in medical social work or case management.

Responsibilities

• Coordinate care for members with long-term rehabilitation or psychosocial needs.
• Conduct assessments for long-term care and disease management programs.
• Develop individualized case plans to prevent poor outcomes.
• Communicate updates with providers and care teams.
• Identify opportunities to ensure care within member benefit plans.
• Provide information on social issues and available services.
• Develop programs to reduce admissions and improve health outcomes.
• Coach junior colleagues in social work techniques.
• Collaborate with community organizations and policymakers.
