Job summary

The role requires assessing members, developing individualized plans, and communicating with clinicians to improve outcomes. Travel within Wayne/Macomb Counties is required; a Social Work degree or related licensure

Qualifications

• Minimum 2 years experience in medical social work or case management.
• Must travel up to 25–50% in Wayne and Macomb Counties with reliable transportation.
• Complies with all state requirements where duties are performed.
• Basic problem solving and decision making skills.
• Basic digital literacy skills.
• Basic knowledge of medical terminology.
• Ability to deal tactfully with customers and community.
• Ability to handle sensitive information ethically and responsibly.
• Ability to evaluate costs and benefits of actions.
• Ability to function in a clinical setting with diverse cultural dynamics.

Responsibilities

• Controls comprehensive management of members with acute or chronic conditions focusing on quality of care and outcomes.
• Performs initial and periodic assessments for long-term care or case management programs.
• Develops individualized case plans for at-risk members.
• Communicates updates with providers, PCPs, Medical Directors, pharmacists, and care management staff.
• Identifies opportunities to ensure members receive necessary care under their benefit plans.
• Provides information and education on social issues, mental health, and available services.
• Develops programs to reduce admissions and improve health outcomes.
• Coaches junior colleagues in social work techniques and processes.
• Collaborates with community organizations to address issues and raise awareness.
