Qualifications

• Master's Degree in Social Work is required by state law.
• LMSW licensure is required.
• Willing and able to travel up to 50% in Wayne and Macomb Counties.
• Basic problem-solving and decision-making skills.

• Ability to handle sensitive information ethically and responsibly.

Responsibilities

• Controls comprehensive management of members with acute or chronic conditions, focusing on quality of care, outcomes and cost reduction.
• Performs initial and periodic assessments for Long-Term Care or case/disease management programs.
• Develops individualized case plans for at-risk members.
• Communicates updates with providers, PCPs, Medical Directors, pharmacists, and care management staff.
• Identifies opportunities to ensure members receive care within their benefit plan in home or community settings.
• Provides information and education on social issues, mental health, and available services.
• Develops programs to reduce admissions and improve health outcomes.
• Coaches junior colleagues in social work techniques and processes.
• Collaborates with community members and organizations to address community issues.
