Indiana Professional Management Group in Mitchell, IN is seeking a Case Manager for a full-time hybrid role that blends remote work with travel to community meetings across Lawrence County. Applicants must reside in Lawrence County and be able to travel for in-person meetings several times a week.

The role focuses on educating clients about Medicaid waivers, developing individualized support plans, coordinating services, and leading cross-agency meetings to ensure care goals are met.

Qualifikationen

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Or Bachelor's degree in an unrelated field with a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
• A valid driver's license, car insurance, and reliable transportation
• The ability to pass thorough background screens
• Designated home office setting with computer, internet service, and smartphone

Aufgaben

• Provide information and education to help individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the individuals we serve
• Help individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
