Job summary

Indiana Professional Management Group, Inc is seeking a full-time Case Manager to serve Lawrence County, IN. The role blends remote work from a home office with travel to attend several community-based meetings weekly within the county and surrounding areas.

You will educate clients about Medicaid waivers, develop individualized support plans, help access needed services, and coordinate with care teams across providers.

Qualifications

• Bachelor's degree in psychology, sociology, social work, or related field, or
• Bachelor's degree in unrelated field with 1 year of relevant experience or 2 years serving aging population.
• Must have valid driver’s license, car insurance, and reliable transportation.
• Must pass background screens and maintain home office setup.

Responsibilities

• Provide information and education about the Medicaid waiver process.
• Create an Individual support plan based on client needs and goals.
• Help Individuals access services to meet their goals.
• Facilitate regular community-based team meetings to review plans with providers.
