Job summary

Indiana Professional Management Group (IPMG) is seeking a Case Manager to work full-time in a hybrid role, combining home-based work with travel to community meetings within Harrison County, Indiana. You will help Individuals understand Medicaid waivers and develop personalized supports to meet goals.

The role emphasizes collaboration with care teams, travel within the county, and scheduling regular team meetings to review plans of care and coordinate services.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation
• Or a Bachelor's degree in an unrelated field with at least one year of direct experience with people with intellectual and/or developmental disabilities, or two years with aging population
• Valid driver's license, car insurance, and reliable transportation
• Ability to pass thorough background screens
• Designated home office with computer, internet, and smartphone

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
