Job summary

IPMG is seeking a Case Manager for a full-time, hybrid role serving Lawrence County, Indiana. The position requires travel within the county for in-person meetings and client coordination from a home office.

Candidates must reside in Lawrence County and be able to travel to surrounding areas as needed. You will educate individuals about Medicaid waivers, develop individual support plans, assist in accessing needed services, and lead regular team meetings with providers to ensure plans are

Qualifications

• Bachelor degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or a related degree
• A valid driver’s license and reliable transportation
• Able to pass thorough background screens
• Home office with computer, internet, and smartphone

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process
• Create an Individual Support Plan based on strengths, needs, goals, and desires of individuals served
• Help individuals access services and supports to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
