Indiana Professional Management Group, Inc. seeks a Case Manager to serve Vigo County, Indiana. This full-time hybrid role blends remote work with travel to community-based meetings several times a week.

You will help Individuals navigate Medicaid waivers, develop individualized support plans based on strengths and goals, and assist them in accessing needed services and supports. You will also facilitate regular community-based team meetings and collaborate with service providers to review plans

Qualifications

• Bachelor's degree in psychology, sociology, social work, nursing, or another human services field.
• Bachelor's degree in an unrelated field plus 1 year direct experience with individuals with intellectual/developmental disabilities, or 2 years with aging population.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office with computer, internet and smartphone.

Responsabilités

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an individual support plan based on strengths, needs, goals and desires of the individuals we serve.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
