Job summary

Travel within Greene County for in-person meetings is required. The position requires a relevant bachelor's degree or equivalent experience, a valid driver's license, and reliable transportation.

Qualifications

• Provide information about Medicaid waiver process to Individuals.
• Create an Individual support plan based on strengths, needs, goals.
• Help Individuals access services and supports to meet goals.
• Pass thorough background screens as part of hiring.

Responsibilities

• Travel within Greene County and surrounding areas for meetings.
• Facilitate regular community-based team meetings to review plans.
• Coordinate with service providers to implement plan of care.
• Maintain home office setup with computer, internet, and phone.

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation

Bachelor's degree in unrelated field + 1 year direct experience or 2 years aging population experience
