Job summary

Indiana Professional Management Group in Lawrence County, Indiana, is seeking a Case Manager for a full-time hybrid role combining remote work with in-person meetings across the county. Applicants must reside in Lawrence County and be willing to travel within the county and surrounding areas.

The role involves developing Individual support plans, educating individuals about Medicaid waivers, and coordinating with providers to connect people with needed services.

Qualifications

• A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• A Bachelor's degree in an unrelated field with at least one year of direct experience with people with intellectual/developmental disabilities, or two years with aging population.
• A valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens and maintain secure home-office setup.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the individuals we serve.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
