Job summary

Indiana Professional Management Group in Hamilton County, IN is hiring a Case Manager for a full-time, hybrid role combining remote work with travel within the county and surrounding areas.

Candidates must reside in Hamilton County, IN and be able to travel to community-based meetings. A Bachelor's degree in a human services field or related experience is required, along with a valid driver’s license and background checks. IPMG offers sign-on bonuses and ESOP ownership.

Qualifications

• Bachelor's degree in the listed human services fields or an equivalent combination of education and experience.
• Direct experience with individuals who have intellectual and/or developmental disabilities or aging populations is required or preferred.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office setup with computer, internet, and smartphone.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an individual support plan based on strengths, needs, goals, and desires of the individuals served.
• Help individuals access services and supports needed to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation or another human services field

Bachelor's degree in an unrelated field with 1+ year experience with individuals with disabilities or 2+ years with aging population
