Indiana Professional Management Group is seeking a full-time Case Manager to serve Hendricks County, Indiana. The role combines remote work from a home office with travel to community-based meetings several times a week.

Candidates must reside in Hendricks County and be able to travel across the county and surrounding areas. A Bachelor's degree in a human services field or related experience is required, along with a valid driver's license and a reliable vehicle.

Qualifications

• Bachelor's degree in a human services field or equivalent experience.
• Valid driver's license and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office with computer, internet and smartphone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
