Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Or a Bachelor's in an unrelated field with at least one year of direct experience with people with ID/DD, or two years with aging population.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office setting with computer, internet, and smartphone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field
