Job summary

Indiana Professional Management Group (IPMG) is seeking a Case Manager in Marion County. This full-time hybrid role blends remote work with travel for community-based meetings several times weekly.

Applicants must reside in Marion County and be able to travel throughout the county for in-person meetings. The role requires a human services Bachelor's degree or relevant experience, a valid driver's license, and the ability to pass background screens.

Qualifications

• Bachelor's degree in human services or related field, or related experience.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office with computer, internet, and smartphone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals, and desires.
• Help Individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review plan of care.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field

Bachelor's degree in an unrelated field and 1+ year direct experience with people with intellectual/developmental disabilities or 2+ years with aging population
