Job summary

Indiana Professional Management Group, Inc. (IPMG) is hiring a full-time Case Manager in Allen County, IN. This hybrid role blends remote work from your home with in-person community meetings, requiring travel within the county and surrounding areas.

You will educate individuals about Medicaid waivers, develop tailored support plans, and coordinate services with providers to help people reach their goals.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
• A valid driver's license, car insurance, and reliable transportation
• The ability to pass thorough background screens
• Designated home office setting with computer, internet service, and smart phone

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Education
