Job summary

Indiana Professional Management Group (IPMG) is seeking a Case Manager for Lake County, Indiana. This full-time hybrid role combines remote work with travel to community meetings within the county and surrounding areas.

Applicants must reside in Lake County and be able to travel for in-person meetings several times a week. You’ll educate individuals about the Medicaid waiver process, build individualized support plans, help access services, and lead community-based team meetings with providers.

Qualifications

• Bachelor’s degree in psychology, sociology, social work, special education or related field.
• Valid driver’s license, car insurance, reliable transportation.
• Background screens are required.
• Home office with computer, internet, and smartphone is needed.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual support plan based on individual strengths, needs, goals, and desires.
• Help individuals access services and supports to meet their goals.
• Facilitate regular community-based team meetings to review plans and coordinate with providers.
