Job summary

IPMG, Monroe County, Indiana, is seeking a Case Manager for a full-time hybrid role that combines remote work from your home with travel within Monroe County and surrounding areas for in-person meetings. You will provide information about the Medicaid waiver process, develop Individual Support Plans, help individuals access services, and facilitate team meetings with service providers.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field.
• Or a Bachelor's degree in an unrelated field with a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.
• A valid driver's license, car insurance, and reliable transportation.
• The ability to pass thorough background screens.
• Designated home office setting with computer, internet service, and smart phone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
