Quality Care LLC in Minnesota is seeking a Care Coordinator / Direct Service Professional to join our team in a hybrid role that blends administrative support with in-home care. You will split time between the Care Coordination team and visiting clients at their homes.

This position requires strong organizational and communication skills, a high school diploma, and a valid driver's license. Prior caregiving is a plus, but not required; reliable transportation and a compassionate demeanor are

Qualifications

• High school diploma or equivalent required.
• Experience with office software and ability to handle paperwork accurately.
• Compassionate, patient, reliable, and detail-oriented.
• Previous caregiving or direct support preferred but not required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Complete paperwork and documentation for client services.
• Support scheduling, communications, and office organization.
• Provide companionship, emotional support, and daily living assistance.
• Help clients develop independent living skills at home.
• Collaborate with Care Coordinators and staff to promote client well-being.
