Job summary

Quality Care LLC in Eden Prairie, MN, is seeking a Care Coordinator / Direct Service Professional to join our team in a hybrid role blending administrative tasks with in-home care. You will work with the Care Coordination team on scheduling and documentation, while also providing companionship and assistance with daily living activities to support client independence.

Qualifications

High school diploma or equivalent required.
Strong organizational and communication skills.
Experience with office software and ability to handle paperwork accurately.
Compassionate, patient, reliable, and detail-oriented personality.
Previous experience in caregiving or direct support preferred but not required.
Valid driver's license and reliable transportation.

Responsibilities

Complete paperwork and documentation related to client services, including data entry and filing.
Support scheduling, communications, and office organization to ensure smooth daily operations.
Provide companionship, emotional support, and assistance with daily living activities such as meal preparation, housekeeping, and transportation.
Help clients develop and maintain independent living skills in their home environment.
Collaborate with other Care Coordinators and staff to promote client well-being.
