Job summary

Quality Care LLC in Shakopee, MN seeks a Care Coordinator / Direct Service Professional to join our team in a hybrid role blending admin tasks with hands-on client care.

You will document services, coordinate scheduling, provide companionship, assist with daily living activities, and collaborate with staff to promote client well-being. A high school diploma and reliable transportation are required; caregiving experience is welcome but not required.

Qualifications

• High school diploma or equivalent required.
• Strong organizational and communication skills.
• Experience with office software and ability to handle paperwork accurately.
• Compassionate, patient, reliable, and detail-oriented personality.
• Previous experience in caregiving or direct support preferred but not required.
• Valid driver''s license and reliable transportation.

Responsibilities

• Complete paperwork and documentation related to client services, including data entry and filing.
• Support scheduling, communications, and office organization to ensure smooth daily operations.
• Provide companionship, emotional support, and assistance with daily living activities such as meal preparation, housekeeping, and transportation.
• Help clients develop and maintain independent living skills in their home environment.
• Collaborate with other Care Coordinators and staff to promote client well-being.
