Quality Care LLC in Minnesota seeks a Care Coordinator / Direct Service Professional to blend administrative duties with in-home care. This hybrid role supports the Care Coordination team and also provides direct client assistance.

You will complete paperwork, coordinate schedules, offer companionship, help with daily living skills, and collaborate with staff to promote client well-being. A high school diploma is required, and strong organizational and communication skills are essential.

Qualifications

• High school diploma or equivalent required.
• Strong organizational and communication skills.
• Experience with office software and ability to handle paperwork accurately.
• Compassionate, patient, reliable, and detail-oriented personality.
• Previous experience in caregiving or direct support preferred but not required.

Responsibilities

• Complete paperwork and documentation related to client services, including data entry and filing.
• Support scheduling, communications, and office organization to ensure smooth daily operations.
• Provide companionship, emotional support, and assistance with daily living activities such as meal preparation, housekeeping, and transportation.
• Help clients develop and maintain independent living skills in their home environment.
• Collaborate with other Care Coordinators and staff to promote client well-being.
