Job summary

Quality Care LLC is seeking a dedicated Care Coordinator / Direct Service Professional to join our hybrid team in Minnesota. This role blends administrative tasks with in-home client support, helping coordinate services while delivering compassionate care.

You will handle paperwork, scheduling, and communications, provide companionship and daily living assistance, and collaborate with staff to promote client well-being. A valid driver's license is required.

Qualifications

• High school diploma or equivalent required.
• Strong organizational and communication skills.
• Experience with office software and ability to handle paperwork accurately.
• Compassionate, patient, reliable, and detail-oriented personality.
• Previous experience in caregiving or direct support preferred but not required.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete paperwork and documentation related to client services, including data entry and filing.
• Support scheduling, communications, and office organization to ensure smooth daily operations.
• Provide companionship, emotional support, and assistance with daily living activities such as meal preparation, housekeeping, and transportation.
• Help clients develop and maintain independent living skills in their home environment.
• Collaborate with other Care Coordinators and staff to promote client well-being.
