Quality Care LLC in Minnesota is seeking a dedicated Care Coordinator / Direct Service Professional for a hybrid role combining office support with in-home care.

You will split time between the Care Coordination team handling documentation, scheduling and communications, and providing companionship, daily living assistance, and transportation support in clients' homes. High school diploma required; strong organization and communication skills are essential.

Qualifications

• High school diploma or equivalent required.
• Strong organizational and communication skills.
• Experience with office software and ability to handle paperwork accurately.
• Compassionate, patient, reliable and detail-oriented.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete paperwork and documentation related to client services, including data entry and filing.
• Support scheduling, communications, and office organization to ensure smooth daily operations.
• Provide companionship, emotional support, and assistance with daily living activities such as meal preparation, housekeeping, and transportation.
• Help clients develop and maintain independent living skills in their home environment.
• Collaborate with other Care Coordinators and staff to promote client well-being.
