Community Housing Innovations, Inc. (CHI) is seeking a Case Manager in Brooklyn, NY to support residents through service and care plans, coordinating referrals and direct services to foster stabilization and independence.

The role emphasizes collaborating with social workers, monitoring progress, and ensuring access to public benefits. HS Diploma required; BA in social work/psychology preferred; salary around $45k annually with a 4-day weekly schedule.

Qualifications

• High School Diploma or GED required; Bachelor’s preferred.
• Three years of experience in shelter or similar field; equivalent education substitution allowed.
• Strong English communication, verbal and written; multi-lingual a plus.

Responsibilities

• Coordinate service and care plans and provide customized information, counseling, crisis intervention, direct services, and advocacy to residents.
• Develop and maintain a resource network for referrals related to residents’ service needs.
• Meet with residents weekly to assess progress and modify care plans as needed.
• Coordinate with Social Workers and Treatment Team to ensure health and safety of residents.
• Design discharge plans to ensure continued enrollment in public benefits and community services.
