Job summary

Community Housing Innovations, Inc. (CHI) seeks a full-time Case Manager at our Coram, NY location to help families achieve independence through housing, employment, and supportive services.

You will assess client needs, develop Independent Living Plans, coordinate referrals, document progress, and support clients with budgeting, resumes, and housing readiness to maintain stable housing.

Qualifications

• Bachelor’s degree in human services or related field; Master’s Degree in Social Work preferred.
• Two years’ experience working in homelessness or housing preferred.
• Excellent verbal and written communication skills; English required; bilingual in Spanish a plus.

Responsibilities

• Adhere to program regulations, policies, and procedures; ensure quality services that meet requirements.
• Assist clients in developing an Independent Living Plan (ILP) and evaluate progress on goals.
• Provide information, referrals, counseling, crisis intervention, direct service, and advocacy.
• Develop and maintain a resource network; collaborate with DSS and other providers.
• Assist clients with budgeting, resumes, ADL skills, and other direct services.
• Conduct face-to-face meetings with clients; inspect living spaces for compliance.
• Document and report housing unit needs and progress; complete discharge plans.
