Job summary

Community Housing Innovations (CHI) is a not-for-profit housing and human services organization focused on stabilizing residents and promoting self-reliance. The Case Manager develops and updates service plans, coordinates direct services and referrals, and monitors resident progress in a supportive environment.

The role requires a high school diploma or GED with preference for a BA in social work or psychology, plus experience in shelters or social services.

Qualifications

• Education: High School Diploma or GED required; Bachelor's degree preferred in social work or psychology.
• Experience: 2+ years in case management or related social services is described.
• English: Proficiency required; multilingual ability a plus.

Responsibilities

• Coordinate service and care plans for residents and link to external services.
• Meet with residents weekly to assess progress and adjust plans.
• Document interactions and ensure enrollment in public benefits and DHS programs.
• Collaborate with social workers and other providers to support stability and independence.
• Conduct required room inspections and track client progress.
